Responsibilities and mandates
Mandates and remits enabling people and organisations to participate in care
- care mandate
  mandate based on a commitment and either an informed consent or an authorization by law defining the rights and obligations of one care actor with regard to their involvement in care processes performed for a specific subject of care
- care commitment
  acceptance of a care request including commitment by the care actor to perform requested care activities
- informed consent
  permission to perform care activities, voluntarily given by a subject of care with consent competence or by a subject of care proxy with consent competence after having been informed about the purpose and the possible results of the care activities
- objection
  subject of care desire that specific care activities are not performed
- consent competence
  capability of the subject of care and/or the subject of care proxy to give or withhold informed consent
- authorization by law
  provision in legislation that in certain circumstances may overrule the need for informed consent
- subject of care desire
  desire expressed by the subject of care or the subject of care proxy regarding the performance of certain care activities
- care activity mandate
  care mandate assigning the right and obligation to perform specific care activities
- clinical care activity mandate
  care mandate to provide needed care activities concerned with the physical or mental health of a subject of care
- social care activity mandate

EXAMPLE 1 A mandate assigned to a coordinating nurse, to a lead General Practitioner, key-worker, etc. to coordinate care

Note 1 to entry: Beyond solely assuming the function described above, a continuity facilitator may also assume the function of a lead and/or coordinator of care activities delivered to the subject of care

Note 2 to entry: A continuity facilitation can be fulfilled only if the involved care actors have the information needed to perform their tasks in care activities

Note 3 to entry: For continuity of care the continuity facilitator mandates for complete clinical processes are of special importance from the subject of care s perspective.
