request for care where a care professional asks a care provider to perform one or more care provider activities 

EXAMPLE 1 Request for a care assessment, an operation, a wheelchair, etc.

EXAMPLE 2 Request for a second opinion where the responsibility remains with the requesting party

Note 1 to entry: A request is put forward by a care professional within a care process

Note 2 to entry: The responsibility for the requested care provider activities is held by the performer but they will be performed under the care period mandate of the requester.

Note 3 to entry: A care provider may accept or decline a request (order) to perform care activities.
