EXAMPLE 1 To increase the subject of care's adherence to the treatment in a hypertension programme.
EXAMPLE 2 To stabilise the subject of care’s systolic and diastolic pressure in a hypertension programme.
EXAMPLE 3 To increase the subject of care’s knowledge of their disease through an educational programme for diabetics.
Note 2 to entry: A care goal could be expressed as one or several target conditions to be reached within a specified date and time.
